NOTICE OF PRIVACY PRACTICES

Harbor Homes, Inc. is required by law to provide you a copy of our privacy practices, legal duties, and your rights concerning your mental and/or physical health information.

How Harbor Homes, Inc. May Use or Disclose Your Mental and/or Physical Health Information

  • For Treatment - We may use or disclose your mental and/or physical health information to provide, coordinate, or manage your mental and/or physical health care treatment between health care providers.
  • For Payment - We may use or disclose your mental and/or physical health information so that services can be billed to you or a third party.
  • As Required by Law - We will disclose mental and/or physical health information when required to do so by federal, state or local law.
  • Health and Safety - Your mental and/or physical health information may be disclosed to avert a serious threat to the health or safety of you or any other person pursuant to applicable law.
  • Government Functions - Your mental and/or physical health information may be disclosed to carry out specialized functions, such as national security or to correctional institutions.
  • Lawsuits and Disputes If you are involved in a lawsuit or dispute, we may disclose mental and/or physical health information about you in response to a court order or administrative order. We may disclose mental and/or physical health information about you in response to a subpoena, discovery request, or other lawful process by someone involved in the dispute, but only if efforts have been made to tell you about the request.

You Have the Following Rights With Regard To Your Mental and/or Physical Health Information

  • You have the right to request restrictions on certain uses and disclosures of your mental and/or physical health information. We are not required to agree to your request for restrictions. Any request for restrictions must be submitted in writing to Harbor Homes, Inc. , stating what information you want limited, to whom you want the limits to apply to, and if you want to limit our use of your mental and/or physicalhealth information.
  • You have a right to inspect and copy your mental and/or physical health information. You must submit a request in writing, along with the reason for the request. We may charge a reasonable fee for the costs of copying, mailing, or other supplies necessary to grant your request. We may deny your request in certain limited circumstances. If your request is denied, you can request a review of that denial.
  • You have the right to receive an accounting of disclosures of your mental and/or physical health information we have made after April 14, 2003 for purposes other than;
    • Treatment and payment for Harbor Homes, Inc.
    • To you, based upon your authorization
    • Certain government functions
  • You have the right to request confidential communication of your mental and/or physical health information. You must submit a written request to have your information mailed to you at a different address. We will accommodate reasonable requests.

How to File a Complaint

If you believe your privacy rights have been violated by Harbor Homes, Inc., you may file a complaint addressed to the HIPAA Privacy Officer, NHDHHS, 129 Pleasant Street, Concord, NH 03301. Your complaint must be in writing. You may also file a complaint with the Secretary of the Department of Health and Human Services at 200 Independence Ave., SW, Washington , DC 20201. You will not be penalized or retaliated against for filing a complaint.

Harbor Homes, Inc. reserves the right to change this notice. We reserve the right to make the revised or changed notice effective for health information we already have about you as well as any information we receive in the future.
